Diamond’s single story design showcases two distinct yet similarly designed residential wings for assisted living and memory care, thereby enabling a smooth transition and familiarity with surroundings when a resident requires memory care. To determine program enrollment, a resident’s accommodation and service options are aseessed upon admission and routinely monitored to ensure the best living arrangement.
Diamond’s Memory Care community is intimate and uniquely designed to address supports and needs due to acute cognitive and memory deficits. Our memory care wing is secure and supervised while offering calming and cozy environs to help attenuate associated wandering tendencies. Residents enjoy renovated units, restaurant style dining and purposefully appointed commons which lead to secure outdoor walking paths and gardens.
Memory Care life-enrichment incorporates multi-modal stimulation utilizing aromas, lighting, sound, exercise, spiritual and emotional interactions in small “doses.” Staff assist residents in memory care specific activities, integrating supervised program participation throughout the community.
Personal care addresses activities of daily living with added and diversional interventions to help redirect wandering. Our wellness team collaborates with resident clinicians and hospice providers to alleviate symptoms and the behavioral challenges associated with cognitive decline.
Diamond’s personal care entails a hybrid of hospitality and support to enhance each resident’s Diamond living experience. Wellness plans are adjusted accordingly as personal care needs change; working with providers and families to determine how to best address needs.
